1.Complete the new patient packet and fax it to our office. a) Be sure to complete the forms with as much detailed as possible. Forms with incomplete information will only delay the process! b )Submit any recent labwork results and/or diagnostic studies (within past 12 months) along with new patient forms. c) When your information is faxed to our office, please call and confirm that was it has been received.
2.Get our comprehensive bloodwork and/or hormone testing results. a) Once your patient history and questionnaires have been received by our office, we will contact you with lab orders to go to a Quest or LabCorp drawing station to get your blood drawn. If you have a PPO insurance, your lab tests may be covered through your insurance plan. For cash paying patients, we have discounted pricing when tests are order through our office. Test results are usually available in 3-5 days. b) Saliva testing (required for hormonal evaluation) is a take-home test that is done in one day, dropped in the mail to the testing company and results are received in 10 days. c) When all results are received, your appointment will be scheduled.
3.Schedule your appointment. a) A deposit is required to make your initial appointment that will be applied to your initial office visit. (See Fees/Services/Office Policies) b) Your initial office visit will be a 1.5- 2 hour appointment. c) Be sure to bring all the medications and supplements you are currently taking to your appointment. Also, please bring the original copies of your forms to your appointment
